Family Medicaid is a federal and state funded program for families, children and pregnant women who meet eligibility criteria. There are two main categories which have different income limits: Healthy Start and Healthy Families. Coverage includes: doctor visits, hospital care, prescriptions, immunizations, prenatal care, vision, dental, substance abuse and mental health services. These programs DO NOT REQUIRE A FACE TO FACE INTERVIEW.
Healthy Start
Healthy Start is for eligible children and pregnant women. Children can be covered from birth to age 19 and pregnant women can be covered up to 60 days after the birth of the baby. Families with private health insurance may still be eligible and families with a higher income may pay a premium for Medicaid coverage.
Expedited Medicaid
Expedited Medicaid provides coverage for pregnant women for 60 days but will not cover hospitalization.. Only verification of pregnancy and a statement of income is required. All verifications will be needed to extend medical coverage for the full term of pregnancy, the birth of the child and 60 days post partum.
Healthy Families
Healthy Families Medicaid provides assistance for low-income families. This category of assistance covers children and their parent(s) or caretaker and has a lower income limit than Healthy Start.
Application Requirements
- Complete and submit the Combined Programs Application
- Provide proof of:
- Income (earned and unearned: SSD, SSI, Child Support, etc.)
- Citizenship**
- Social Security Numbers
- Residency - You must apply in your county of residence

** Medicaid Citizenship Documentation
In order for you to receive (or continue receiving) Medicaid coverage, US citizenship must be verified. The law requires that original document(s) be provided to the agency; no copies will be accepted.
You must provide one of these original forms:
- US Passport
- Certificate of Naturalization (N-570)
- Certificate of US Citizenship (N-560 or N-561)
If you do not have one of the forms listed above, one original item from each category listed below must be provided:
Birth Document
- US Birth Certificate
- Certificate of birth abroad
- Hospital Record of Birth
- Request Assistance From ACFS
Identification
- Driver's license or State ID
- Government issued ID
- US Military ID
- School Picture ID if currently in school
- Affidavit signed by a parent or legal custodian
- If under age 16, school record, report care of child care/nursery school record
